IT IS SAFER THAN ENDOVASCULAR STENT IMPLANTATIONS AND AORTIC SURGERIES

Before endovascular aneurysm repair (EVAR) was introduced to the clinical practice, “thoracoabdominal aortic an­eurysm surgeries” have been difficult for both patients and surgeons and associated with high mortality and com­plication rates. For those aneurysms that develop in patients, who are usually at advanced age or have serious co­morbidites such as heart, lung and kidney conditions, left intercostal and/or median abdominal incisions would have been made. Those conventional surgery techniques were not only taking long hours, but they were also accompanied by very long hospital stays and conditions related with all organs.

 

In EVAR method, general anesthesia is induced and patient is transferred to the angiography room for interven­tion. Intervention starts with a 4- to 5-cm skin incision and preparation of artery(ies) at the inguinal locus. Surgical intervention ends with this procedure and next, a synthetic woven fabric that is wrapped around the stent is placed through guide wires that are advanced through vessels, such that aneurysm is completely covered along with a safe zone before and after the aneurysmatic segment. Stent is deployed in the aorta, while it is located between two ends of aorta with normal calibration.

 

Accordingly, blood circulation in the aneurysmatic, dissected or even ruptured aortic segment is bypassed. At the same time, blood supply is supplied to end organs through the lumen of the stent.

 

ADVANTAGES OF ENDOVASCULAR TREATMENT

EVAR procedure takes far shorter time and lesser skin incision is made. No bone is divided and patients’ recovery time is significantly shorter as they are less traumatized and less volume of blood is lost. Hospital stay is generally shorter than three days. Complication rare and mortality rate are very significantly lower relative to the conventional technique.

Who are eligible for underarm surgeries?

Irrespective of young or old, obese or lean or female or male patient, it can be performed for any patient who has a heart valve problem or requires repair of a cardiac hole.

Who are eligible for underarm surgeries?

Heart valve is exposed with intercostal approach, after a skin incision, measuring 6 to 8 cm, is made parallel to the breast line in the right armpit. In many cases, size of incision can be further shortened using endoscopic imaging methods and tools. Surgery can be performed in an easier, more comfortable and safer manner with video-assisted endoscopic surgery method.

What PROs of the armpit cardiac surgery?

• Postoperative recovery is faster.

• Stay in the ICU and hospital shortens.

• Postoperative need to blood transfusion decreases.

• Postoperative pain is milder and patient can lie down in bed in any position.

• Not being obliged to lie down in supine position for a long time particularly for patients, who are obese or has sleep apnea or who cannot lie down or sleep in supine position, is a great comfort.

• Patient may get out or get in the bed independently or without requiring any help, and may cough easily.

• This technique is free of risk of sternal dehiscence (separation of breastbone), which is a serious risk for osteo­porotic and obese patients, since sterna bone is not divided.

• Postoperative surgical wound healing is quicker.

• Patient is discharged early.

• Patient may resume daily routines easily and quickly right after the surgery.

• Better cosmetic outcomes are achieved. It is particularly preferred by young and female patients, since its es­thetic quality is higher.

OBESITY CENTER

Efficient outcomes are obtained in surgeries carried out with laparoscopic method in the Obesity Center of Medistate Hospital Obesity. Most appropriate surgical method is applied using cutting-edge technology and multi-disciplinary approach in conjunction with experience and specialization in the field of obesity (bariatric) surgery.

Since laparoscopic method is used for obesity surgeries performed under supervision of Prof. Emin Ersoy, M.D., President of the Turkish Association of Endoscopic and Laparoscopic Surgery, smaller incision and quicker return to social life are ensured.

 

WHICH METHODS ARE USED IN OBESITY SURGERY?

Gastric banding, sleeve gastrectomy, gastric by-pass and gastric plication surgeries are used in obesity surgery. A diet program is prepared in pre-operative period in order to reduce body fat percentage, conserve muscle tissue and make patient ready for post-operative treatment.

 

√ Gastric Band

Gastric banding surgeries are done with laparoscopic method. A small gastric pouch is created with a band placed 3 to 4 cm caudal to the cardia. Since the band is adjustable, tension can be increased or decreased in response to patient’s condition. A subcutaneous port is placed to make adjustments. Band migration, band erosion, difficulty swallowing, damage to band and port insertion problems are among possible postoperative conditions.

√ Sleeve Gastrectomy

Sleeve gastrectomy is the most preferred modality among obesity surgeries. Surgery can be successfully carried out with laparoscopic method. A large portion of the stomach is surgically removed and about 25-30 percent of total vol­ume of stomach is left. Gastric fundus, the superior part of the stomach, is also surgically removed. Therefore, level of ghrelin hormone, also known as “hunger hormone” that is secreted from gastric fundus decreases and appetite is more easily regulated. Although gastric capacity is decreased in this surgery, gastric functions are not impaired. In addition, risk of gastric ulcer reduces. When it is compared to long-term outcomes of gastric by-pass surgeries, weight loss rates are same. This is the surgical modality that causes the least impairment in our physiology.

 

√ Gastric By-pass (Roux-Y)

In gastric by-pass surgery, a pouch, measuring approximately 25-30 cc in volume, is formed at cardia and it is con­nected to a segment of small intestine at 100- to 150-cm distance to the stomach. Surgery restricts food intake, while it also decreases absorption of nutrients. Long-term outcomes are similar to that of sleeve gastrectomy; therefore, sleeve gastrectomy is, recently, somewhat more preferable. Possible postoperative complications are dumping syndromes and ulcer formation at the new passageway. Probability of vitamin deficiencies is higher in comparison to other surgeries.

 

√ Mini-gastric By-pass

This is another type of gastric by-pass surgeries; since this surgery is performed under the name of gastric-bypass, it does not, in fact, draw attention. Unlike usual gastric by-pass surgeries, 1- A longer and larger gastric volume is created. 2- Small intestinal bypass allows biliary and pancreatic enzymes pass through the stomach to intestines. (loop small intestine). In the long run, reflux disease and conditions caused by bile in the gastric lumen are probable.

 

√ Gastric Plication

Stomach wall is folded inwards on the greater curvature, starting from the dome to the gastric outlet. It reduces volume of the stomach. There is no risk of leakage or bleeding, since no incision is made. It has gradually been popu­lar. It is usually preferred as secondary surgery for patients, who has already undergone gastric banding surgery. Outcomes are similar to that of sleeve gastrectomy in experienced centers.

 

Laparoscopic technique is used for all surgeries in the Obesity Center of Medistate Hospital and open surgeries are avoided, unless it is absolutely indicated.  The most suitable method is determined for each patient by the surgeon in the light of preoperative laboratory and radiologic examinations.

Medical Oncology

Medical Oncology is a medical branch that deals with all anti-cancer medication therapies. Chemotherapy is an umbrella term for “medication therapies”. Recently, chemotherapy is an important treatment modality which in­creases the chance of success in almost all types of cancer, especially breast, lung, colon, ovarian cancer, as well as lymphoma and leukemia.

 

While chemotherapy alone is used for some cancer types, it is applied in combination with other treatment methods, such as surgery and radiotherapy, for some other types. Chemotherapy aims to increase chance of cure in some types of cancers by destructing the tumor and preventing the spread and to kill rest cancerous cells in postoperative period in some other and to maximize curative effect of radiotherapy in others. In our hospital, professional oncolo­gists not only determine the most accurate personalized treatment, but they also take necessary measures to mini­mize the damage on the healthy organs secondary to this therapy and to protect the quality of life.

 

Radiation Oncology

In Department of Radiation Oncology, ionizing radiation is used to take malignant tumors under control as a part of cancer treatment. Basic principle of radiation therapy is to kill cells or to prevent their proliferation. Radiotherapy aims primarily to give the highest dose planned for the tumor, while peri-tumoral healthy tissues are exposed to the smallest dose possible.

 

Devices, such as TrueBeam that are developed to deliver the radiation to the diseased organ in the most accurate and appropriate way, also known as Linear Accelerator, are required for this purpose. “Radiotherapy” or “radiation therapy” implies using those devices to deliver high-dose radiation in order to destruct cancerous tissue or to ensure palliation.

SURGICAL ONCOLOGY

Conventional surgeries and minimally invasive procedure, which play an important role in the treatment of cancer, are performed in the field of cancer surgery.

 

In our Department of General Surgery, surgeries are carried out for breast cancer and benign breast disorders, esophagus cancer and its benign diseases, stomach cancer, colorectal tumors, anorectal tumors, benign and ma­lignant tumors and cysts of liver, tumors of gall bladder and biliary ducts and cysts and cancers of pancreas and thyroid cancer.

 

In the Gynecologic Oncology Unit of the Department of Gynecology and Obstetrics, laparoscopic procedures are car­ried out for the treatment of cervical cancers, ovarian cancers, vulvar-vaginal cancers and trophoblastic tumors.

In the Department of Neurosurgical Oncology; head and neck cancers and tumors of brain stem are managed us­ing surgical modalities. Metastatic brain tumors, glial tumors, meningiomas, pituitary adenomas and spinal cord tumors are the most common conditions that are managed with surgical modalities in the Department of Neurosur­gical Oncology.

 

Multidisciplinary approach has a great importance also for treatment of urological cancers. Experienced Urologists perform surgeries for kidney cancer, testicular cancer, urinary bladder cancer and other urologic oncology condi­tions.

 

NUCLEAR MEDICINE

Nuclear medicine is a branch of medical imaging that uses small amounts of radioactive material to diagnose and determine the severity of or treat a variety of diseases, including many types of cancers, heart disease, gastrointesti­nal, endocrine, neurological disorders and other abnormalities within the body. Highly simplified, a typical procedure would involve discussing the procedure with the patient, injecting the radiopharmaceutical which localizes in the area of interest, then is imaged using a special camera. It is like taking an X-ray from the inside-out.

ALGOLOGY (PAIN TREATMENT)

The majority of people with cancer will experience pain at some time or another. The pain can result from the cancer itself, or from the cancer’s treatment. In addition, some people who have been cured of their cancer can continue to suffer from pain. Although cancer pain is usually treated with medicine, surgery to remove a tumor or radiation therapy to shrink a tumor can be used along with medicine to provide additional pain relief. In most cases, doctors treat cancer pain with pain-relief medicines called analgesics or with non-drug treatments such as physical therapy and rehabilitation, imagery, biofeedback and relaxation techniques. Other treatment options for cancer patients in­clude nerve blocks, which involve the injection of pain medicine into or around a nerve or the spine.

The Pediatric Oncology Branch is dedicated to improving outcomes for children and young adults with cancer and genetic tumor predisposition syndromes. It is thought to be one of the most challenging of specialties. There are many medical professionals who devote themselves to some aspect of this specialty. These could include the many medical technicians that may perform or aid in treatment as prescribed by a pediatric oncologist.

Pediatric oncology, while it may concern itself most with eradication of cancerous illness, may also need reflect on how best to preserve a mentally healthy atmosphere for the child through providing parental support as possible. TrueBeam STx is a cutting-edge and new-generation radiation therapy device. TrueBeam STx system utilizes many new techniques with its completely restructured control system for imaging, patient positioning, movement man­agement and dynamical synchronization of treatment. It can be used in all advanced radiotherapy modalities in­cluding conventional and 3D conformal radiotherapy as well as image-guided radiotherapy and radiosurgery (IGRT and IGRD), intensity modulated arc therapy (RapidArc) and stereotactic body radiotherapy (SBRT). Intracranial and extracranial radiosurgery methods are carried out with a precision of lower than 1 mm with this system.

Brachytherapy

Brachytherapy is an irradiation technique,where radioactive sources are placed in body spaces or between tissues (temporarily).

 

Risk of complication in healthy tissues decreases since it is applied at high doses before or after external therapy to a more local region to ensure a better tumor control, while lower doses are delivered to the vulnerable tissue around the tumor. Brachytherapy alone can also be used to ensure cure in appropriate situations.

 

Brachytherapy is an effective treatment method, which is commonly used for cervical, prostate, breast and skin cancers. Brachytherapy can be used for tumors in other body part or in other words, it is not limited with the above mentioned regions. Examples of those body parts include tongue base, floor of mouth, lip, bronchus, nasopharynx, esophagus and rectum.

 

PET/CT

PET/CT is an imaging method that combines PET (Positron Emission Tomography) and CT (Computed Tomography) devices to get metabolic and anatomic data of organs concomitantly.

 

Advantages of PET/CT

PET/CT helps differentiation of benign or malignant lesions.

It prevents unnecessary biopsy and surgical procedures.

It helps to determine spread of cancer in body or in other words, to stage the tumor with a single examination.

It is the most accurate method to identify recurrence of cancer.

It enables to evaluate response of some types of cancers to chemotherapy agents. Therefore, it makes possible to switch the ineffective therapy to the efficient one.

It helps to evaluate response to treatment following radiotherapy and chemotherapy.

 

Gamma Camera

Gamma camera converts photons spreading from radionuclide, which is placed in the patient, into a pulse and then a tension signal. This signal is used to form an image of radionuclide’s distribution. Gamma cameras are like a kind of camera. They do not radiate beams. Gamma cameras convert photons of gamma beams that are radiated by human body just like conversion of natural light into photons by cameras.
